Hi Ariv,

Please refer to 3 of the screenshots that I uploaded.

Screenshot 2, I created a field called Application Method, and under Choose a field type, I chose Checkbox. Under Manage Options, those fields are Spray, Dust, Bait and Other which was shown correctly as per Screenshot 1.

However, when user ticks checkbox for Other , user will also need to specify what “other” is, and user needs to enter a description as short text. See the example of the hard copy as per screenshot 3.

I can only choose one type of field type, this could be working as it is. Is there a way on how I can add a short text to Other checkbox?
